Important Test Kit Information

Please note, if you are a resident of California, Maryland or New York:

California residents: California State health law does not allow the testing of specimens collect or mailed from California without a written order from a health care professional licensed to prescribe in California.

Maryland and New York State residents: State health laws in Maryland and New York prohibit the processing of internet orders. Please contact ZRT Laboratory Customer Service at 1-866-600-1636 for more information.

All Testers:

ZRT is a CLIA certified lab that provides the service of collecting and analyzing specimen samples. You will be submitting health information and ordering tests for informational purposes only. ZRT has a contracted licensed physician who will review your request and authorize the testing service you are purchasing. You are responsible for taking the results of your testing to a provider who is qualified in diagnosing and treating any condition through your use of this service.

Insurance Reimbursement:

If you are wanting your test kit lab fees to be covered by your insurance, you must FIRST schedule an appointment with our Nurse Practitioner (or a medical provider of your choosing). She will assist you in the selection of the best test kit for you based on your medical history and current symptoms and concerns. In this way, appropriate ICD-9 diagnostic codes can be added to your lab requisition, which will validate the CPT test codes being ordered.

Choosing a Medical Provider:

If your test results indicate that you have a hormonal imbalance (either too high or too low), you will need a medical provider to prescribe appropriate treatments to bring you back into balance. If bio-identical hormones are prescribed, your levels will need to be monitored to make sure your dosages are correct. Hormone balancing can greatly improve the quality of your life.

As soon your new patient paperwork is received (either on-line or through the postal service), a medical consult can be scheduled. Our Nurse Practitioner will review your complete medical history, assess any health risks you may have, discuss your symptoms and concerns, answer all your questions regarding treatment options, and help you order the saliva or blood spot test kit that is best for you.

Once the results of your labs are received, our Nurse Practitioner will go over your test results with you. If your hormones are out of balance, a prescription for bio-identical hormones will be ordered through a compounding pharmacy. Kathryn will make recommendations for you to optimize your health through various modalities. She may recommend additional lab tests, based on your medical history and health risk factors.
